本プライバシーポリシーは、日本の「個人情報の保護に関する法律」（APPI）に基づき制定されています。当店は利用者の個人情報を、視力検査の予約・眼鏡のオーダー・コンタクトレンズの販売・問い合わせ対応・法令遵守の目的に限り取得・利用いたします。

14. 個人情報の第三者提供

ご本人の同意がある場合、または法令に基づく場合を除き、取得した個人情報を第三者に提供することはありません。レンズの加工を外部に委託する場合は、適切な秘密保持契約のもと業務委託先を管理いたします。

15. 個人情報の開示・訂正・削除

ご本人またはその代理人からのご請求により、当店が保有する個人情報の開示・訂正・追加・削除・利用停止・第三者提供の停止に応じます。お問い合わせ窓口宛にご連絡ください。原則として2週間以内に対応いたします。

16. 視力検査データの取り扱い

店内で実施する視力検査の結果は、お客様の処方箋作成と次回の検査履歴管理にのみ利用します。当該データは厳重に保管し、第三者に提供することはありません。お客様のご希望によりデータの開示・削除が可能です。

当店では個人情報保護管理者を設置し、個人情報の安全管理および従業員教育を実施しています。お問い合わせは下記窓口までお願いいたします。
